Collected Personal Data

All personal data pertaining to client is for maintaining records for legal and ethical reasons.

The client is not compelled to use their legal names. They may use a pseudonym. The client will not be consulted as anonymous. The client must be 18 years and older. The client must provide information relating to age, in the form of full date of birth. All clients must provide Informed Consent prior to engaging in consultations. Disclosure of Identity number is not essential. The Counsellor does not take legal and ethical responsibility and accountability for any client or user on the website who uses fraudulent means of access to services. Privacy and Confidentiality will be maintained at all times. No incriminating records will be kept by the   Counsellor. The client takes full responsibility and accountability for the decisions they make. Every effort will be made to ensure that Wellness Counselling promotes Positive living and Positive outcomes.

Purpose of collecting data

Data is collected for Mental State Examination and formulating Nursing Diagnosis. Data is utilized for statistical purposes and to maintain billing records for tax purposes.
